A string of repeated, preventable near misses involving humans a problem
The mines inspectorate has again called on mine operators to replace low-order administrative risk controls with higher-order risk management, such as an engineered solution, isolation, substitution, and, where possible, elimination, following a string of preventable high potential incidents.
Collisions involving mobile plant were once again identified as a problem, mostly involving haul trucks.
